Health close to home

REACH is, together with Lemvig Municipality and DEFACTUM, a partner in the project Sundhed I det Nære (Health close to Home), which develops and tests community-based initiatives aimed at strengthening wellbeing, participation, and agency in Harboøre and in a social housing area in Lemvig.

The project focuses on how to create more equitable access to health services in areas characterised by long geographical distances and significant social health challenges. In Harboøre and in a social housing area in the town of Lemvig, everyday health-promoting initiatives are being developed and tested in close partnership between the municipality, citizens, civil society organisations, and knowledge institutions.

The initiatives combine open community-based activities with targeted support for citizens living with chronic illness or at increased risk. Through a co-creative and systemic approach, citizens’ experiences are linked with organisational learning to ensure that the project not only establishes new activities but also strengthens long-term and sustainable forms of collaboration for local prevention.

The project is funded by the Danish Health Authority and will run until 2028.
